Job summary

CVS Health is seeking a Case Management Coordinator to support members in Kankakee, Iroquois, Livingston, Ford, Champaign and nearby counties. The role emphasizes applying critical thinking to coordinate care and help members access benefits and education using care management tools.

You will evaluate member needs, develop and monitor care plans, identify high risk factors, coordinate with clinical teams and providers, and ensure compliant documentation.

Qualifications

• Bachelor's degree in behavioral health or human services (psychology, social work, marriage and family therapy or counseling).
• 2 years of experience in behavioral health, social services or related field.
• Case management and discharge planning experience preferred.
• Proficiency with standard corporate software (Word, Excel, Outlook, PowerPoint).

Responsibilities

• Evaluate members' needs/eligibility and recommend care solutions.
• Coordinate and monitor assigned care plan activities.
• Identify high risk factors and facilitate referrals to clinical care management.
• Engage with members to promote health and lifestyle changes.
• Document care activities following regulatory guidelines.
