Job summary

CVS Health in Illinois is seeking a Case Management Analyst to coordinate care for members across Zion, Waukegan, Beach Park and surrounding areas. This full-time role requires up to 75% travel and a focus on evaluating needs, coordinating care plans, and supporting member education.

The ideal candidate has at least 2 years in behavioral health or human services, a degree in a relevant field, and strong communication and coaching skills.

Qualifications

• Minimum 2 years of experience in behavioral health or human services.
• Minimum 2 years of case management experience.
• Willing to travel up to 50-75% to meet members in IL.
• Reliable transportation; mileage reimbursed per policy.

Responsibilities

• Coordinate and implement care plan activities and monitor progress.
• Evaluate member needs and eligibility and refer to appropriate programs.
• Provide coaching and support to empower healthy lifestyle choices.
• Collaborate with case managers and Medical Directors to resolve issues.
