Job summary

Molina Healthcare is seeking a Nurse Practitioner to provide screening, preventive and medical care in non-clinical settings such as in-home, community, and nursing facilities. You will perform evaluations, coordinate care, and develop medical plans for diverse patient populations, collaborating with physicians as needed.

The role involves arranging visits, coordinating post-discharge plans, and traveling within the assigned region up to 120 miles daily.

Qualifications

• At least 1 year of experience as a nurse practitioner.
• Active and unrestricted Family Nurse Practitioner license.
• DEA license with authority to prescribe.
• Current BLS certification.
• Valid driver's license with reliable transportation.
• Ability to work across diverse settings and populations.
• Strong communication and time-management skills.
• EMR proficiency.

Responsibilities

• Provides general medical care and care coordination to adult, women’s health, pediatric, and geriatric populations.
• Performs comprehensive evaluations including history and physical exams for gaps in care and preventive assessments.
• Orders diagnostic tests and develops care plans with physician oversight as needed.
• Creates and implements a medical plan of care and documents it timely in the EMR.
• Performs face-to-face visits in-home, nursing facilities, and community locations.
• Drives up to 120 miles daily within the assigned region and may travel further for special projects.
• Obtains and maintains cross-state licenses as needed by business requirements.
• Collaborates with other nurse practitioners to develop best practices and participate in regional meetings.
• May prescribe medications and perform procedures as appropriate.
