Job summary

Complete Care in New Jersey is seeking an experienced External Case Manager to develop referral partnerships and drive patient transitions into post-acute and long-term care. This field-based role focuses on building relationships with hospitals, case managers, and other referral sources to generate qualified referrals.

You will educate patients and families about available services, coordinate with admissions teams, and travel regularly within New Jersey to partner with facilities and providers

Qualifications

• Healthcare sales, business development, admissions, case management, or clinical liaison experience required.
• Skilled nursing, post-acute, rehabilitation, hospital, home health, or hospice experience preferred.
• Knowledge of the hospital discharge and patient-placement process.

Responsibilities

• Build relationships with hospitals, case managers, social workers, physicians, and other referral partners.
• Generate referrals for Complete Care facilities.
• Educate patients, families, and referral sources about available services.
• Coordinate with admissions teams to support timely referral review and placement.
• Conduct regular visits to hospitals and community partners.
• Track outreach, referrals, and market activity.
• Collaborate with admissions, clinical, and operational leadership to support census growth.
