Job summary

Complete Care, Inc. is seeking an experienced External Case Manager to support our New Jersey facilities.

This field-based role focuses on developing referral partnerships, generating qualified referrals, and helping patients transition into post-acute and long-term care. You will build relationships with hospitals, case managers, social workers, physicians, and other referral sources; educate patients and families about available services; and coordinate with admissions teams to support timely

Qualifications

Healthcare sales or case management experience required.
Experience in building referral partnerships and physician/hospital relationships preferred.
Strong communication and relationship-building skills are essential.
Travel within New Jersey is required to cover field-based responsibilities.

Responsibilities

Build relationships with hospitals, case managers, social workers, physicians, and other referral partners.
Generate referrals for Complete Care facilities.
Educate patients, families, and referral sources about available services.
Coordinate with admissions teams to support timely referral review and placement.
Conduct regular visits to hospitals and community partners.
Track outreach, referrals, and market activity.
Collaborate with admissions, clinical, and operational leadership to support census growth.
