Job summary

Complete Care in New Jersey is seeking an experienced External Case Manager to support our skilled nursing facilities. This field-based role focuses on developing referral partnerships, generating qualified referrals, and helping patients transition into post-acute and long-term care.

The ideal candidate will have healthcare sales or case management experience, strong communication and relationship-building skills, and willingness to travel within New Jersey to meet partners and facilities.

Qualifications

• Experience in healthcare sales, business development, admissions, case management, or clinical liaison.
• Experience in skilled nursing, post-acute care, rehabilitation, hospital, home health, or hospice preferred.
• Knowledge of the hospital discharge and patient-placement processes.
• Strong communication and relationship-building skills.
• Reliable transportation and ability to travel regularly within New Jersey.
