Job summary

Complete Care, based in New Jersey, seeks an experienced External Case Manager to support our skilled nursing facilities. This field-based role focuses on developing referral partnerships, generating qualified referrals, and helping patients transition into post-acute and long-term care.

The ideal candidate will have healthcare sales or case management experience, strong communication skills, and willingness to travel regularly to hospitals and community partners in New Jersey.

Qualifications

• Experience in healthcare sales, business development, admissions, case management, or clinical liaison roles.
• Experience in skilled nursing, post-acute, rehabilitation, hospital, home health, or hospice preferred.
• Knowledge of hospital discharge and patient-placement processes.
• Excellent communication and relationship-building skills.
• Reliable transportation with regular travel within New Jersey.

Responsibilities

• Build relationships with hospitals, case managers, social workers, physicians, and other referral partners.
• Generate referrals for Complete Care facilities.
• Educate patients, families, and referral sources about available services.
• Coordinate with admissions teams to support timely referral review and placement.
• Conduct regular visits to hospitals and community partners.
• Track outreach, referrals, and market activity.
• Collaborate with admissions, clinical, and operational leadership to support census growth.
