Job summary

BCForward is seeking a Community Engagement Navigator (Contract) for Elevance Health in Asheville, NC. The role involves in-person work 4–5 days weekly, engaging with members to address Social Determinants of Health, setting goals, and coordinating with providers and referral sources.

Requirements include a public health, social work, or related degree and at least two years in a related field. Travel, multitasking, and strong communication are essential.

Qualifications

• Bachelor's degree in Public Health, Social Work, Rehabilitation Counseling, or related field.
• Minimum of two years of experience in marketing, job development, job placement, social work, or related discipline.
• Knowledge of Vocational Rehabilitation preferred.
• Ability to travel to worksites as required.
• Equivalent combinations of education and experience will be considered.

Responsibilities

• Engage with members to remove SDOH barriers.
• Establish goals and next steps with members.
• Assist in selecting providers or programs most appropriate for supporting their needs.
• Interface with HCM to align individualized plans with case management.
• Act as liaison between referral sources and the health plan.
• Multi-task with calls, texts, facsimiles, and electronic queues while taking notes.
• Maintain focus and productivity in a fast-paced, high-volume environment.
