BCForward is seeking a Community Engagement Navigator (Contract) for Elevance Health in Asheville, NC. The role involves in-person work 4–5 days weekly, engaging with members to address Social Determinants of Health, setting goals, and coordinating with providers and referral sources.

Anticipated Start Date: 10/5/2026

Responsible for engaging with members to remove barriers to Social Determinants of Health (SDOH) and acting as the communication liaison between the company and referral sources.

This position requires regular in-person engagement with patients, members, and healthcare providers, with approximately 4 to 5 days per week spent in the field or on-site.

Primary duties may include, but are not limited to:

Engage with members who have been identified as possessing significant SDOH barriers.

Work with the member to establish goals and next steps for addressing SDOH.

Support member in selecting provider or programs most appropriate for supporting their needs.

Interface with HCM to ensure that the individualized plan is aligned with existing or needed case management efforts.

Act as a liaison between referral sources and the health plan.

Associates in this role are expected to have the ability to multi-task, including handling calls, texts, facsimiles, and electronic queues, while simultaneously taking notes and speaking to customers.

Maintain sustained focus and productivity while performing duties that require extended periods of sitting.

Effectively manage multiple priorities in a fast-paced, high-volume, and deadline-driven environment.

Demonstrate strong verbal and written communication skills, engaging professionally with stakeholders through both virtual and in-person interactions.

Apply exceptional attention to detail, critical thinking, and analytical problem-solving skills to address complex situations and drive effective outcomes.

Exhibit empathy, professionalism, and persistence when addressing inquiries, ensuring issues are resolved thoroughly and efficiently.

Leverage digital tools, systems, and technology platforms to optimize productivity, streamline processes, and reduce manual effort.

Consistently deliver high-quality work while balancing accuracy, efficiency, and organizational objectives.

Adhere to a structured work schedule while maintaining flexibility to support occasional overtime and evolving business requirements.

Demonstrate the ability and willingness to work on-site as business needs dictate and support operational effectiveness.

Performs other duties as assigned.

Requirements:

Bachelor's degree in Public Health, Social Work, Rehabilitation Counseling, or a related field.

Minimum of two (2) years of experience in marketing, job development, job placement, social work, or a related discipline.

Equivalent combinations of education and experience that provide the required knowledge, skills, and abilities will be considered.

Knowledge of Vocational Rehabilitation (Voc Rehab) principles and practices is preferred.

Ability to travel to worksites and other locations as required to support business and client needs.

Preferred:

Experience working with children in foster care or former foster care or adoption.

Experience with Microsoft Office to include Microsoft Outlook, Microsoft Excel, and Microsoft Teams.

Unless specified as primarily virtual by the hiring manager, contractors are required to work at an Elevance Health location at least once per week, and potentially several times per week.
