Responsibilities

• Manage member's individual service plan as required by state law/contract
• Conduct telephonic or face-to-face history and needs assessments
• Identify members with potential clinical health care needs and coordinate with clinical team
• Manage non-clinical needs of members with chronic illnesses to ensure cost-effective utilization
• Establish short and long-term goals with member, caregivers, family and physicians
• Identify members who would benefit from expanded services
