Job summary

Elevance Health in Cary, NC is seeking a Community Engagement Navigator (Contract) to help members overcome Social Determinants of Health barriers. You will engage with members, coordinate with referral sources, and ensure the individualized plan aligns with case management efforts in a hybrid work setting.

The role requires a BA/BS and at least one year of relevant experience; field work 4-5 days weekly; strong communication, empathy, and multitasking skills; travel to worksites as needed and

Qualifications

• Requires BA/BS degree and a minimum of 1 year of experience working directly with people related to the program population or other related community-based organizations; or any combination of education and experience which would provide an equivalent background.

Responsibilities

• Engage with members to remove barriers to Social Determinants of Health (SDOH) and act as the communication liaison between the company and referral sources.
• Engage with members identified as having significant SDOH barriers; establish goals and next steps for addressing SDOH.
• Assist members in selecting providers or programs aligned with needs; interface with HCM to ensure plans align with case management efforts.
• Act as a liaison between referral sources and the health plan.
• Multitask across calls, texts, faxes, and electronic queues while taking notes and speaking to customers.
• Work in a fast-paced environment with strong verbal and written communication; demonstrate empathy and problem-solving to resolve issues.
• Structured schedule with occasional overtime or flexibility; some work from the office as needed.
• Field work 4-5 days per week in-person with patients/members/providers; travel as necessary.
