Job summary

HealthFlex is seeking a Field Clinical Supervisor to blend hands-on PT practice with field supervision, training, and quality initiatives. You will lead a team of PTs, PTAs, OTs, and STs, conduct Start of Care evaluations, and ensure high-quality care while keeping patients safely at home.

Qualifications

• California PT license in good standing
• Current CPR certification
• 1–2 years home health experience preferred; supervisory/mentoring experience preferred
• Familiarity with Medicare COP, OASIS and CA home health rules
• Proficiency with WellSky and Paylocity (or willingness to train)
• Valid CA driver’s license and transportation
• Strong written and verbal communication; ability to mentor staff

Responsibilities

• Provide field supervision for PTs, PTAs and other clinicians.
• Deliver hands-on training and onboarding for new hires.
• Conduct SOC evaluations and field visits.
• Lead annual performance evaluations and contribute to supervision.
• Oversee scheduling, documentation, and QA activities.
• Support root-cause analysis for rehospitalizations and education initiatives.
