Job summary

CVS Health Corporation is seeking a Case Management Coordinator for a full-time, field-based telework role in Ohio. You will travel within Northeast Ohio to member homes and locations, coordinating care and providing support to ensure optimal health outcomes.

Residents in Medina County and surrounding areas are preferred. The role requires at least a Bachelor's degree or a non-licensed master level clinician credential, 2+ years in behavioral health or child-focused services, and solid

Qualifications

• Bachelor's degree or non-licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience in children's mental health, child welfare, or related public sector field.
• 2+ year of experience with Ohio delivery systems, local networks and resources.
• Willing and able to travel within the assigned region up to 50% of the time.
• Reliable transportation; mileage reimbursed per policy.

Responsibilities

• Collaborate with care teams to facilitate appropriate healthcare outcomes for members.
• Provide care coordination, support, and education to members and their families.
• Utilize care management tools and resources to monitor progress and outcomes.
• Act as a single point of contact for members and address gaps in care.
