CVS Health is seeking a Case Management Coordinator in Ohio for a full-time field-based telework role. Travel up to 50% within Northeast Ohio to member homes and locations.

The position requires a Bachelor’s degree or non-licensed master level clinician with 2+ years in behavioral health and related services. Strong communication, care coordination, and inclusive, culturally competent care are essential.

Qualifications

• Bachelor’s degree or non-licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with MS Office Suite applications (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children’s mental health, child welfare, developmental disabilities, juvenile justice, or public sector human services.
• 2+ years experience in areas: family systems, community systems and resources, case management, child and family counseling/therapy, child protection, or child development.
• 2+ year of experience with Ohio delivery systems, including local community networks and resources.

Responsibilities

• Coordinate and implement care plan activities and monitor progress.
• Evaluate referred members’ needs/eligibility and determine care approaches.
• Collaborate with multidisciplinary teams to overcome barriers to goals.
• Provide coaching, information, and support to empower members in healthcare decisions.
• Serve as the single point of contact for members and address gaps in care.
