Qualifications

• RN licensure preferred; or bachelor’s degree in health or human services with CCM/CDMS/CRC.
• Valid driver’s license and reliable transportation.
• High speed internet required; ability to work remotely with travel.
• Motor vehicle record check may be required.

Responsibilities

• Serve as patient advocate to guide and coordinate care for injured workers and families.
• Assist patients in recovery and autonomy through advocacy, communication, and education.
• Identify providers and facilities across services for timely, cost-effective care.
