Job summary

CVS Health is seeking a Case Management Analyst for the Danville, VA region. This field-based role focuses on LTSS care coordination, supporting members with evaluation, planning and education to achieve optimal health outcomes.

The ideal candidate has 2+ years in behavioral health or related fields, is based in Danville or nearby, and is able to travel up to 75% with mileage reimbursed. A clinical degree or certification is preferred.

Qualifications

• 2 years’ experience in behavioral health, social services or related field
• Must reside in Danville, Virginia or surrounding areas
• Willingness and ability to travel up to 75% of the time; mileage reimbursed

Responsibilities

• Collaborate to inform the case management process and provide care coordination, support and education to members.
• Evaluate member needs/eligibility and recommend approaches to case resolution.
• Coordinate and monitor care plan activities and progress.
• Escalate quality of care issues and use negotiation to secure appropriate options and services.
• Engage members through motivational interviewing to support healthy lifestyle choices.
• Document care and outcomes in accordance with regulatory guidelines and company policies.
