Qualifications

• 2 years' experience in behavioral health, social services or related field
• Must reside in Danville, Virginia or Surround Areas
• Must travel up to 75% of the time; mileage reimbursed

Responsibilities

• Apply critical thinking to inform case management and coordinate care for members using management tools and resources
• Evaluate member needs/eligibility and recommend an appropriate care plan
• Identify risk factors affecting outcomes and refer to appropriate clinical teams
• Coordinate and monitor care plan activities and progress
• Collaborate with Medical Directors and other programs to improve care quality
• Escalate quality of care issues through established channels
• Engage members using motivational interviewing to support lifestyle changes
• Provide coaching and information to empower member decision-making
• Assist members in partnering with providers in healthcare decisions
• Utilize case management and quality processes in compliance with regulatory guidelines
