Job summary

CVS Health Corporation's LTSS team seeks an Analyst, Case Management to support members with care coordination, education, and resource navigation. This field-based role operates out of Danville, Virginia, with travel up to 75% to meet members face to face.

You will assess needs, coordinate care plans, and collaborate with clinicians to overcome barriers while ensuring regulatory compliance and high-quality care.

Qualifications

• 2 years' experience in behavioral health, social services or related field.
• Must reside in Danville, Virginia or Surround Areas.
• Must travel up to 75% of the time; mileage reimbursed per policy.

Responsibilities

• Collaborate to inform the case management process and assist with care coordination for members.
• Evaluate member needs/eligibility and recommend case resolution approaches.
• Identify risk factors, coordinate care plan activities and monitor progress.
• Escalate quality of care issues and use motivational interviewing to engage members.
• Provide coaching and support to empower informed health decisions.
