Qualifications

• 2+ years' experience in behavioral health, social services or related field.
• Willing to travel up to 75% of the time.
• Must reside in Annandale, Falls Church, Springfield, Arlington, or Alexandria VA area.

Responsibilities

• Face-to-face and telephonic assessments, planning and coordinating all case management activities with members.
• Evaluate medical needs and develop proactive plans to improve short- and long-term wellness.
• Coordinate services in line with network management and clinical coverage policies.
