Job summary

Enlyte is seeking a Field Case Manager to coordinate care for injured workers in a hybrid role based in El Monte, CA. You will partner with treating physicians, employers, and patients to create effective treatment plans and ensure cost-efficient, quality care.

The ideal candidate is an RN with 2+ years of clinical practice and familiarity with workers' compensation case management. Travel to in-person appointments is required, with a strong emphasis on independent workload management and

Qualifications

• Active RN license in good standing.
• 2+ years clinical practice preferred.
• URAC-recognized case management certification (CCM, CDMS, CRC, CRRN or COHN, COHN-S, RN-BC, ACM, CMAC, CMC).

Responsibilities

• Develop and implement treatment plans to return injured workers to work.
• Collaborate with physicians/providers and employers to tailor care.
• Assess recovery progress and adjust case goals as needed.
• Manage workload and prioritize cases for timely outcomes.
• Attend injured workers' appointments when required and provide updates.
