Job summary

The People Concern's Santa Monica C3 Team is seeking a case manager to join our multidisciplinary street outreach team. This role supports a City of Santa Monica strategy to identify, engage, and connect homeless individuals to interim or permanent housing and supportive services.

The Case Manager will help clients develop individualized service plans, enroll in benefits, submit housing applications, and access medical and behavioral health treatment, while prioritizing wellness, recovery and

Qualifications

• Knowledge of homelessness, mental illness, substance use, medical issues, and domestic violence.
• Computer literacy; ability to use a computerized database system for information management.
• Valid CA driver’s license, reliable car, auto insurance, and acceptable driving record.

Responsibilities

• Engage and provide field-based services as part of an interdisciplinary team.
• Represent the agency with partners to reduce homelessness.
• Develop mutual goals with clients and document in service plans.
• Provide initial contact, needs assessment, information and referrals.
• Refer clients to health, mental health, interim housing, and other services.
• Collaborate to remove barriers to permanent housing (ID, benefits, life skills).
• Provide case management and aftercare for clients in permanent housing.
• Help clients build a supportive peer community.
• Work with public, private and nonprofit partners to offer outreach services.
• Adhere to customer choice, harm reduction, and psycho-social rehabilitation.
• Maintain complete, accurate member files and funder-required documents.
• Ensure data in the case management database is up to date.
