Special Service for Groups, Inc. is seeking an Outreach Case Manager in Santa Monica, CA, to perform field-based case management for homeless consumers.

The role involves assessing needs, developing treatment plans, coordinating services, and documenting progress with a multidisciplinary team. The incumbent will work evenings possibly, in a non-exempt, full-time capacity, and will engage with a range of health, housing, and support services to facilitate client stabilization and linkages.

Qualifikationen

• Some college credits or two years of experience working with homeless individuals
• Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds
• Experience in field outreach and coordination with multidisciplinary teams
• Knowledge of Microsoft Word, Excel and Internet; data/documentation experience
• Background check and ability to obtain/t maintain clearances and HIPAA awareness

Aufgaben

• Conduct immediate needs assessments and develop comprehensive treatment plans in MDT context
• Provide field outreach to connect homeless individuals with benefits, housing and health services
• Document daily activities in HMIS and maintain HIPAA compliant records
• Coordinate transportation and liaison with community agencies to support clients
• Attend required meetings, trainings, and supervision sessions
