Qualifications

• 2+ years experience in Behavioral Health, Social Services or related field.
• Willingness to travel 50-75% within the Metro East, Illinois area.
• Experience with electronic documentation and MS Office.

Responsibilities

• Evaluate members and assess needs/eligibility and plan care.
• Identify high risk factors and refer to appropriate case management.
• Coordinate and monitor care plan activities and progress.
• Support care quality and engage members in decision-making.
