Job summary

Institute for Community Living Inc. (ICL) is seeking a role within a community-based team to support individuals in maintaining stable, independent living.

You will conduct home visits, provide counseling, coordinate services, and advocate for consumer needs in the community. The role requires coordinating with care managers, performing crisis interventions, and assisting with access to medical, social, educational, vocational, and financial resources.

Qualifications

• Experience providing direct services to individuals with mental health needs.
• Counseling, case management, crisis intervention, and service coordination experience.
• Willingness to travel within the community; driver’s license may be preferred.

Responsibilities

• Conduct home visits to provide supportive counseling and independent living assistance.
• Develop individualized service plans after assessing needs.
• Coordinate services with care managers, ICL staff, and external providers.
• Advocate for consumer needs within ICL and with external providers.
• Perform crisis assessment and intervention; refer to appropriate services.

Bachelor’s degree in human services

Associate’s degree in human services + 2 years experience

High school diploma/GED + 4 years experience
