Job summary

Right at Home Kansas City area is seeking a Care Team Specialist (CTS) to support clients, mentor caregivers, and ensure high-quality service delivery. This field-based leadership role will be hands-on, assisting with client care, caregiver training, emergency staffing, client visits, and quality assurance activities.

The CTS will model professionalism, adhere to policies, and maintain a team-oriented attitude while coordinating with the Scheduling Manager and Client Services teams.

Qualifications

• Current CNA certification preferred.
• Combination of education and experience totaling at least three years in healthcare, home care, or a related field preferred.
• Current on all certifications, training requirements, and continuing education required by Right at Home and the State.

Responsibilities

• Deliver services per each client's care plan.
• Provide direct client care as needed.
• Fill open shifts due to call-offs, emergencies, and urgent staffing needs.
• Communicate client concerns and changes in condition to office staff and update the plan of care accordingly.
• Provide hands-on caregiver training and mentoring in home and office training spaces.
• Support caregiver retention and participate in retention initiatives.
• Follow company policies and promote a positive, collaborative team environment.
• Assist with emergency staffing needs and on-call rotations.
• Complete documentation accurately and on time.
• Maintain confidentiality and HIPAA compliance.
