Job summary

Molina Healthcare in Naperville, IL is seeking a care coordination professional to support management and coordination across the care continuum, ensuring member progress toward outcomes and contributing to cost-effective, high-quality care strategies.

You will assess members, develop and implement care plans with members and the care team, conduct visits, monitor progress, and coordinate services across behavioral health, LTSS, and community resources, including travel as required.

Qualifications

• Minimum 2 years healthcare experience in care management or related setting.
• Valid driver's license and reliable transportation may be required.
• Knowledge of community resources and care coordination best practices.
• Ability to work independently with minimal supervision.

Responsibilities

• Completes assessments and determines eligibility for care coordination.
• Develops and implements care plans with members and care teams.
• Conducts telephonic, in-person, or home visits as required.
• Monitors progress and updates care plans to meet goals.
• Maintains ongoing member caseload and coordinates services across behavioral health and LTSS.
• Facilitates interdisciplinary ICT meetings and collaborative planning.
• Assesses barriers to care and assists members accordingly.
• Local travel up to 25-40% as required.
