Job summary

Molina Healthcare in Illinois seeks a care coordination professional to support member care across the continuum. You will conduct assessments, develop care plans with the member, caregiver, physician and care team, and perform telephonic, in-person, or home visits as needed.

The role involves monitoring progress, managing a caseload, promoting integration of behavioral health, LTSS, and community resources, and facilitating ICT meetings while using motivational interviewing to motivate change.

Qualifications

• Strong verbal and written communication skills.
• Excellent time-management and prioritization skills.
• Ability to work independently with minimal supervision.
• Proficiency with Microsoft Office and applicable software.

Responsibilities

• Complete member assessments within timelines and identify care coordination needs.
• Develop and implement care plans with member, caregiver, physician and care team.
• Conduct telephonic, in-person, or home visits as required.
• Monitor and adjust care plans based on progress and interventions.
• Maintain an ongoing member caseload with regular outreach.
• Promote integration of behavioral health, LTSS, and community resources to enhance continuity of care.
• Facilitate interdisciplinary care team meetings and collaboration.
• Use motivational interviewing to educate and motivate change.
• Identify barriers to care and assist members accordingly.
• Collaborate with licensed care managers as needed.
• Travel 25-40% locally as required.
