Qualifications

• At least 2 years in health care, care management or related setting.
• Registered Nurse with active, unrestricted license in state of practice.
• Valid driver's license and reliable transportation for travel.
• Understanding of EMR and HIPAA requirements.
• Knowledge of community resources and care coordination concepts.
• Ability to work independently and manage multiple tasks.

Responsibilities

• Completes comprehensive member assessments within regulatory timelines.
• Develops and implements care coordination plans with member and care network.
• Performs telephonic, home visits or in-person contacts as needed.
• Monitors care plan effectiveness and documents interventions.
• Maintains ongoing member caseload and outreach.
• Promotes integration of behavioral health, LTSS and community resources.
• Facilitates ICT meetings and interdisciplinary collaboration.
• Uses motivational interviewing to support behavior change.
• Identifies barriers to care and provides assistance as needed.
• May provide consultation and resources to peers; complex cases may be assigned.
