The position requires a Bachelor's in Social Work or related field, 3+ years in care coordination/behavioral health, and willingness to travel.

Qualifications

• Bachelor's degree in Social Work or Human Services or related field.
• 3+ years in care coordination, behavioral health, or healthcare setting.
• 3+ years working with medically complex members and communication barriers.
• 1+ years using MS Office (Word, Excel, Outlook).
• Driver's license and ability to travel within territory.

Responsibilities

• Engage members to complete comprehensive needs assessments.
• Develop and implement person-centered care plans for chronic conditions and wellness.
• Collaborate with internal care team and community resources to implement plans.
• Provide education and coaching for self-management and healthy lifestyle changes.
• Support discharge planning and coordinate care transitions.
• Advocate for members and families with the health care team.
