Job summary

UnitedHealthcare is hiring a Field Care Coordinator to travel within the Danville, VA area to assess needs, create care plans, and coordinate with the care team. This field-based role combines in-home assessments with community and facility collaboration to promote self-management and continuity of care.

You will engage with enrollees across facilities and home settings, advocating for patients while adhering to guidelines. A valid driver's license and ability to travel are required.

Qualifications

• Bachelor's degree in Social Work or Human Services (or related field).
• 3+ years of experience in care coordination, behavioral health, or healthcare environment.
• 3+ years of experience with members with medical needs, elderly, disabilities, or communication barriers.
• 1+ years of MS Office experience (Word, Excel, Outlook).
• Driver's license and ability to travel within assigned territory.

Responsibilities

• Engage members face-to-face and/or telephonically to complete comprehensive needs assessments.
• Develop and implement person-centered care plans addressing chronic conditions, wellness, social determinants, medication management and safety.
• Partner with interdisciplinary team, providers, and community resources to implement care plans.
• Provide education and coaching to support member self-management of care needs and lifestyle changes.
• Support proactive discharge planning and coordinate care transitions after ER visits or SNF admissions.
• Advocate for members and families to ensure needs and choices are represented by the care team.
