Qualifications

At least 2 years experience in health care, preferably in care management.
Clinical licensure and/or certification required only if state contract requires.
Valid driver's license with reliable transportation.
Knowledge of community resources.
Ability to work independently with minimal supervision.
Excellent verbal and written communication skills.
Experience with Microsoft Office.
In some states, a bachelor's degree in a health care related field may be required.

Responsibilities

Completes assessments of members per regulated timelines and determines eligibility for care coordination.
Develops and implements care plans with member, caregiver, physician and care team.
Conducts telephonic, face-to-face or home visits as required.
Monitors care plans, documents interventions and evaluates goal achievement.
Maintains ongoing member caseload and regular outreach.
Promotes integration of services across behavioral health, LTSS, and home/community resources.
Facilitates interdisciplinary care team meetings and collaboration.
Uses motivational interviewing and Molina clinical guideposts to support change.
Assesses barriers to care and provides coordination assistance.
Collaborates with licensed care managers as needed.
Travel 25-40% locally depending on state/ contract requirements.
