Qualifications

• 2+ years in health care, preferably care management or equivalent.
• Valid driver's license and reliable transportation.
• Proactive, detail-oriented, and able to work independently.
• Strong communication, both verbal and written.

Responsibilities

• Completes assessments within regulated timelines to identify care coordination needs.
• Develops and implements care plans with member, family, and providers.
• Conducts telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness and documents progress and changes.
• Maintains ongoing member caseload and outreach.
• Promotes integration of behavioral health, LTSS, and home/community resources.
• Facilitates interdisciplinary care team meetings and collaboration.
• Uses motivational interviewing and Molina clinical guidelines to support change.
• Identifies barriers to care and assists members to address concerns.
• Collaborates with licensed care managers as needed.
• Travel up to 25–40% based on state/contract requirements.
