Qualifications

• Active RN license in state of practice.
• Strong proficiency with Microsoft Office (Outlook, Excel, OneNote, Teams).
• Excellent verbal and written communication abilities.
• Must have reliable transportation and valid driver’s license; travel 25–40%.

Responsibilities

• Complete comprehensive member assessments within required timelines, including in-person visits.
• Develop and implement care plans in collaboration with members, caregivers, physicians and the care team.
• Monitor care plans and adjust interventions to achieve goals and ensure quality outcomes.
• Coordinate LTSS and waiver services, ensuring continuity of care across settings.
• Facilitate team meetings and educate members to support behavior health and access to resources.
