Qualifications

• At least 2 years in health care with care management or similar role.
• RN licensed and active in the practice state.
• Valid driver license and reliable transportation for local travel.
• Proficient in Microsoft Office applications and documenting care.
• Ability to work autonomously and coordinate with multidisciplinary teams.

Responsibilities

• Complete comprehensive member assessments, including home visits as required.
• Develop and implement care plans with members, families, and clinicians.
• Monitor care plans, document progress, and adjust as needed.
• Coordinate waiver enrollment/disenrollment and liaise with ICT meetings.
• Educate members on services and motivate behavior change.
• Identify barriers to care and address psycho-social and medical obstacles.
