Job summary

Molina Healthcare is seeking a Texas-licensed RN to join as a Field Care Manager in Fort Worth to assess members for services and support care planning. Preference for candidates with Medicaid/MCO experience; mileage is reimbursed and travel within 30–45 minutes of the coverage area is expected.

The role focuses on coordinating LTSS, validating needs, and working with a multidisciplinary team to improve outcomes while controlling costs. Hours are 8–5 CST, Monday through Friday.

Qualifications

• 2+ years in health care with care management or managed care experience.
• RN license active and unrestricted.
• Driver's license and reliable transportation for travel requirements.
• Strong communication and organizational skills.
• Experience with diverse populations and LTSS is a plus.

Responsibilities

• Complete comprehensive member assessments within timelines, including home visits.
• Coordinate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members and care teams.
• Monitor care plan effectiveness and adjust as needed.
• Promote integration of behavioral health and LTSS services.
• Assess medical necessity and authorize waiver services.
• Participate in ICT meetings and document interventions.
• Conduct medication reconciliation as needed.
• Travel 25–40% may be required.
