Job summary

Molina Healthcare is seeking an RN Field Care Manager for the Tyler, TX area to assess members, coordinate LTSS services, and support care planning. The role requires in-person visits, travel within a 25–40% local radius, and collaboration with a multidisciplinary team.

Ideal candidates have Medicaid/managed care experience, strong Microsoft Office skills, and the ability to work independently in a fast-paced environment. Monday–Friday, 8–5 CST, with mileage reimbursement offered.

Qualifications

• Active RN license in Texas or eligible to practice in the state.
• Minimum 2 years in health care with 1 year in care management or related field.
• Experience with Medicaid, managed care, or LTSS is preferred.
• Valid driver’s license and reliable transportation for local travel.
• Proficiency in Microsoft Office; ability to toggle between programs and complete forms.

Responsibilities

• Complete comprehensive member assessments within required timelines.
• Coordinate waiver enrollment and disenrollment processes.
• Develop and implement member care plans with stakeholders.
• Monitor care plans and document interventions and outcomes.
• Promote integrated care across behavioral health and LTSS.
• Participate in ICT meetings and support care team decisions.
• Use motivational interviewing to educate and support member changes.
• Travel locally 25–40% as required.
