Qualifications

• Active RN license in Virginia with no disciplinary action.
• Two years of healthcare or case management experience.
• One year working with Cardinal Care Priority Population.
• Knowledge of community health and social services.
• Proficient in Word, Outlook and Excel; able to use EMR.
• Willingness to travel 50-75% for field work.

Responsibilities

• Review and analyze for medical complexity.
• Develop member individualized care plans.
• Apply evidence-based strategies to manage health.
• Identify and resolve barriers to care.
• Monitor progress through assessments and evaluations.
• Possibly cover ABD population, depending on assignment.
• Follow department guidelines and procedures.
