Job summary

This role involves performing assessments, coordinating waiver services, traveling as needed (25–40%), and using motivational interviewing to engage members and caregivers while maintaining high-quality, cost-effective care.

Qualifications

• RN license active in state of practice.
• Minimum 2 years health care experience with at least 1 year in care management/LTSS.
• Valid driver’s license and reliable transportation for travel.
• Ability to work independently and manage multiple projects.
• Strong verbal and written communication skills.

Responsibilities

• Complete comprehensive member assessments within required timelines, including in-person home visits.
• Develop and implement care plans with members, caregivers, physicians and others.
• Coordinate waiver enrollment/disenrollment and monitor progress.
• Promote integration of LTSS and behavioral health services for continuity of care.
• Facilitate ICT meetings for service approval/denial and collaboration.
• Perform medication reconciliation as needed and address barriers to care.
