Job summary

Molina Healthcare is seeking a licensed behavioral health clinician to perform care management and coordination across the continuum. The role involves assessing members, developing integrated care plans with their teams, and facilitating care through home visits or telehealth as needed.

Travel up to 25-40% may be required depending on state contracts. Strong communication and proficiency with clinical platforms are essential to support quality and cost-effective member care.

Qualifications

• 2+ years health care experience, preferably behavioral health.
• Active license (LCSW/LMSW/CHES/LPC/LPCC/LMFT/PhD/PsyD) or equivalent, active/unrestricted.
• Valid driver’s license and reliable transportation for travel.

Responsibilities

• Complete behavioral health assessments within timelines.
• Develop and implement care plans with members and providers.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plans and adjust interventions as needed.
• Maintain caseload and coordinate with interdisciplinary teams.
