Qualifications

• Licensed behavioral health clinician with active license in state of practice.
• Valid driver's license and ability to travel 25–40%.

Responsibilities

• Complete behavioral health assessments per timelines and determine eligibility for care coordination.
• Develop and implement care plans with member, family, and care team.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plans and document interventions and progress toward goals.
• Promote integration of behavioral health with LTSS and community resources.
• Facilitate care team meetings and coordinate care across disciplines.
