Job summary

UnitedHealthcare is seeking a Field Care Coordinator to support LTSS and integrated care across Wayne and Macomb Counties, MI. This field-based role includes home visits, a strong focus on person-centered planning, and collaboration with care teams to ensure timely, cost-effective services.

The role requires a social work degree with licensure, experience in community health and social determinants of health, and the ability to travel.

Qualifications

• Must possess Master’s degree and licensure as Social Worker (LMSW/LCSW) or Bachelor’s with licensure.
• 2+ years of experience in community health in a healthcare setting.
• 1+ year of experience with local behavioral health providers addressing SDoH.
• 1+ year of experience with long-term care needs or home- and community-based services.
• 1+ year of experience using electronic documentation systems and MS Office.
• Access to reliable transportation and valid US driver’s license; willingness to travel within assigned territory up to 75%.

Responsibilities

• Develop and implement care plans with a single point of contact.
• Communicate health information to stakeholders for coordinated care.
• Advocate for patients to ensure needs are represented by the healthcare team.
• Assess and plan individualized care strategies at the least restrictive level.
• Identify barriers to care and provide appropriate care management.
• Refer to social services programs (financial, housing, transportation, etc.).
• Provide resource support for local services based on assessments.
• Manage the person-centered service plan across the continuum of care.
• Conduct home visits and in-person visits as required.
