Job summary

UnitedHealthcare is seeking a Field Care Coordinator to operate in Winchester, VA, conducting in-home assessments and developing person-centered care plans in partnership with an interdisciplinary team.

The role requires travel within assigned areas, strong advocacy for enrollees, and collaboration to ensure comprehensive, cost-effective patient care. Remote-friendly arrangements are available within a field-based structure.

Qualifications

• Bachelor's degree in Social Work or Human Services (or related field).
• 3+ years of care coordination or behavioral health experience.
• 3+ years of experience with members who have medical needs, the elderly, or those with disabilities.
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Access to reliable transportation and a valid US driver's license with ability to travel within territory.

Responsibilities

• Engage members to complete needs assessments across medical, behavioral, functional, cultural, and socioeconomic factors.
• Develop and implement person-centered care plans for chronic conditions, promotion, and social determinants of health.
• Collaborate with internal care team and providers to implement care plans.
• Provide education and coaching to support member self-management and healthy lifestyle changes.
• Support care transitions and discharge planning after ER visits or SNF admissions.
• Advocate for members and families to ensure needs are represented.
