Job summary

UnitedHealth Group is seeking a Field Care Coordinator for Blount County, TN. This field-based role focuses on coordinating complex member care across acute, home, chronic, and community settings.

You will apply clinical knowledge to manage cases, educate members, and collaborate with Case Management, Utilization Review, and Behavioral Health teams. The position offers flexibility to work remotely within commutable distance to Blount County, with up to 75% travel to meet members and providers.

Qualifications

• Bachelor’s degree in Social Work, Nursing or other health care profession including LPN.
• 2+ years of clinical case management experience.
• Proficiency with MS Office (Word, Excel, Outlook).
• Excellent organization, communication and time management skills.
• Ability to travel up to 75% within an assigned territory to meet with members and providers.
• Residency in or commutable distance to Sevier County, TN.

Responsibilities

• Apply clinical knowledge to coordinate case management for Medicare/Medicaid beneficiaries.
• Incorporate evidence-based practices into care coordination activities.
• Deliver member-centered, individualized self-management support and education.
• Ensure appropriate utilization and consistent health benefit application.
• Conduct telephonic and in-person assessments per policy and timelines.
• Serve as advocate and facilitator to resolve barriers to care.
• Provide education and coordinate community resource referrals.
• Develop and maintain a member-centered care plan with providers and families.
• Collaborate with Care Coordination Team to improve efficiency and care transitions.
• Monitor hospital encounters and provide education/interventions as needed.
