Qualifications

• At least 2 years experience in health care, care management, or related setting.

• Valid driver’s license with reliable transportation.
• Knowledge of community resources.
• Ability to work independently with minimal supervision.
• Strong communication and time-management skills.
• Ability to manage multiple projects and adapt to change.
• Proficiency with Microsoft Office.
• In some states, a bachelor’s degree in a health care related field may be required.

Responsibilities

• Complete assessments of members per regulated timelines and determine qualification for care coordination.
• Develop and implement care plans with member and care team to address needs and goals.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plan effectiveness and document interventions.
• Maintain ongoing member caseload with regular outreach.
• Promote integration of behavioral health, LTSS, and home/community resources.
• Facilitate interdisciplinary care team meetings.
• Educate and motivate change using motivational interviewing and Molina guides.
• Address barriers to care and assist members accordingly.
• Collaborate with licensed care managers as needed.
• Travel 25-40% locally as required.
