Qualifications

Responsibilities

• Provides support for care management/care coordination activities with multidisciplinary team.
• Develops and implements care plans with member, caregiver, and healthcare professionals.
• Conducts telephonic, face-to-face or home visits as required.
• Monitors care plan effectiveness and documents interventions.
• Maintains ongoing member caseload and outreach.
• Promotes integrated services across behavioral health, LTSS, and home/community resources.
• Facilitates care team meetings and coordination.
