Job summary

Molina Healthcare is seeking a Care Coordinator to support care management activities and collaborate with a multidisciplinary team to coordinate member care across the continuum.

You will complete assessments, develop and implement care plans with the member, caregiver, physician and care team, and conduct telephonic, in-person or home visits as required. Travel up to 40% may be needed; you will promote service integration to improve outcomes.

Qualifications

• At least 2 years experience in health care, care management, or related setting.
• Clinical licensure and/or certification required only if state contract mandates.
• Valid driver's license, reliable transportation, and auto insurance for travel.

Responsibilities

• Completes member assessments per regulated timelines to determine care coordination eligibility.
• Develops and implements care plans with member, caregiver, physician, and care team.
• Conducts telephonic, in-person, or home visits as required.
• Monitors and documents progress, adjusting interventions and goals as needed.
• Maintains ongoing member caseload with regular outreach and management.
• Promotes integration of services across behavioral health, LTSS, and community resources.
• Facilitates ICT meetings and collaboration with the care team.
• Uses motivational interviewing to educate and motivate change during contacts.
• Assesses barriers to care and coordinates support to address concerns.
