Job summary

Humana Inc. is seeking a Transition/Custodial Prevention Specialist to work with members in the field, creating personalized care plans and coordinating services to improve health outcomes and independence.

The role involves extensive travel to member homes or facilities, building relationships with care teams, and guiding members through LTSS options and Medicaid processes. A strong background in social services and Medicaid is required.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or related field.
• Two years of experience with Medicare/Medicaid recipients or HCBS.
• One+ year of custodial/transitional care experience.
• Experience with electronic information applications/software including EMR systems.
• Ability to travel regionally to meet members and families.

Responsibilities

• Meet members in their homes or NF, assess goals and needs, and develop plans of care.
• Coordinate LTSS and connect members to HCBS and community resources.
• Advocate for member care and accessibility to behavioral health services.
• Collaborate with care teams and providers to support independence in the community.
• Educate members on Medicaid eligibility and navigate state systems.
• Maintain member records and enter Level of Care assessments.
• Engage interdisciplinary teams to address housing, social, and health needs.
