Job summary

UnitedHealthcare is seeking a Field Care Coordinator- HIDE SNP to coordinate person-centered, goal-oriented care for LTSS recipients. This field-based role involves home visits, community referrals, and collaboration with care teams to ensure effective, timely services within a 50-mile radius.

Flexible telecommuting is available for Kalamazoo County residents; travel is typical and focused on delivering coordinated care with cost-effective outcomes.

Qualifications

• Master's degree with licensure as a Social Worker (e.g., LMSW/LCSW/LLMSW).
• Bachelor's degree with licensure as a Social Worker (LLBSW/LBSW).
• 2+ years in community health care and 1+ year with SDoH activities.

Responsibilities

• Develop and implement care plan interventions as a single point of contact.
• Communicate health information to stakeholders for coordinated care.
• Advocate for patient needs with the health care team.
• Assess and plan individualized care strategies at the appropriate level.
• Identify barriers to care and provide management interventions.
• Refer to social services including housing, transportation, and financial aid.
• Provide resource support for local services based on assessments.
• Manage the person-centered service plan across the care continuum.
• Conduct home visits and in-person visits as needed.
