Qualifications

• Master's degree and valid independent social work licensure.
• 2+ years’ experience in community health care settings.
• Experience with LTSS and home/community services is preferred.
• Proficiency with MS Office and electronic health records systems.

Responsibilities

• Develop and implement care plans as a single point of contact.
• Coordinate with health providers, patients, and families for timely services.
• Advocate for patient needs within the health care team.
• Plan and adapt care strategies to the least restrictive level.
• Identify barriers to care and initiate appropriate interventions.
• Refer to social services for housing, transportation, and financial aid.
• Maintain person-centered service plans and conduct home visits.
• Document all care activities in compliance with regulations.

Education

Master's degree in Social Work

LCSW / LMSW / LLMSW licensure

Bachelor's degree in Social Work

2+ years community health experience

1+ years with LTSS / home and community-based services

1+ years in MS Office / electronic documentation systems
