Job summary

Elevance Health is seeking a CFSP Managed Care Coordinator (Alleghany County, NC). This field-based role travels to client sites 4–5 days weekly and supports Medicaid-enrolled families through CFSP services, coordinating across physical and behavioral health, I/DD, and LTSS.

A BA/BS and NC residency are required, with travel as needed. The role partners with County Child Welfare Workers and multidisciplinary teams to align health care with permanency goals and ensure trauma-informed, integrated

Qualifications

• Requires BA/BS degree and a minimum of 1 year of experience working directly with people related to the CFSP population or related community-based organizations.
• Experience working in child welfare or Medicaid-related programs is preferred.
• Must reside in North Carolina and be able to travel to worksites as needed.

Responsibilities

• Provide integrated whole-person Care Management under the CFSP Care Management model across physical health, behavioral health, I/DD, LTSS, pharmacy and related services.
• Coordinate care with multidisciplinary teams and County Child Welfare Workers to align health care needs with permanency goals.
• Conduct assessments via telephonic or face-to-face interactions to identify and manage member program needs.
• Identify members who may benefit from expanded services and coordinate their cases with clinical teams.
• Oversee non-clinical needs of members with chronic illnesses to ensure cost-effective, efficient service utilization.
